COCFM's ReCODE Program

We provide world-class ReCODE and functional medicine care. We work closely with you to discover the underlying contributors to your cognitive issues and formulate/revise a plan to help stop and even reverse the progression of your dementia. You will learn how to make the ReCODE lifestyle changes part of your daily routine. We aim to provide you with as much assistance as you need for success. Different people will have different underlying issues; some may require deeper investigation and more visit time than others.

RESToRE 180 Program

Your 6-month Restore 180 Program investment is $9,492 ($1,582/month).

If paid upfront, this is discounted to $8,992, a savings of $500.

Clients must maintain an active ReCODE account with Apollo Health. This is NOT included in your COCFM membership.

NO GUARANTEE

We will do our best to support you in your ReCODE journey. However, we cannot guarantee results. Results are individual and based on how advanced you are at the onset of our work together and on your ability to follow the program outlined by COCFM.

HEALTH COACHING

Health coaching is vital to make lasting health changes and succeed in the ReCODE program.

We have health coaching sessions built into this 6-month program. You will meet one-on-one with your ReCODE 2.0 Certified Health Coach every other week during these 180 days. If you have a ReCODE 2.0 Certified health coach you are working with, you can continue with them (with a decrease in the overall price of the program). However, we expect you to meet with them for at least 30 minutes every other week while you are a Restore 180 member.

Your ReCODE work with COCFM begins with the 6-month Restore 180 program.

This 180-day program is designed to provide you with a high level of support so you can achieve the best outcomes possible.

It requires a commitment to yourself and your future.

The following are included in this 6-month program:

A 30-minute onboarding call to orient you to the program.
30-minute one-on-one health coaching calls every other week (12 calls total).
Three 1-hour visits with Dr. Dan LaPerriere.
A 1-hour weekly community Zoom meeting with Rachel and/or Dr. Dan. These happen at 11 am MST every Thursday (holidays excluded).
Access to our Restore 180 video library. You will be asked to watch a short video once daily to keep you motivated and engaged in the program.
Membership in our ReCODE community forum where you can interact and learn from one another.
Quarterly cognitive assessments through Creyos.
20% off supplements purchased through COCFM.
A substantial discount on peptide/specialty medications purchased through COCFM.
Personal MD -Concierge Communication (see below).
Discounted pricing for metabolic testing done at COCFM.

ADDITIONAL PHYSICIAN TIME

You may schedule additional physician time (beyond the 4 hours paid for in this agreement) at the rate of $450 per hour.

Billable/Deducted time includes all time spent preparing for your visit (review of records, review of questionnaires, review of labs), the visit time itself, time spent composing your visit summary/recommendations, and any follow-up patient portal/email correspondence that requires more than 10 minutes of response time. Time is billed in 15-minute increments.

There is no additional charge for prescription refills or for brief correspondence (that takes less than 10 minutes to respond to).

* Dr. Dan typically spends an extra 30-60 minutes for each hour of face-to-face time in preparation for your visit, making recommendations, placing orders, and composing your notes/plans.

ADDITIONAL HEALTH COACH TIME

You may schedule additional face-to-face health coach time (beyond the hours paid for in this agreement) at the rate of $125 per hour.

OTHER INFORMATION

Labs and Supplements are NOT included in your membership.
To ensure high-quality supplements, we recommend you purchase them through COCFM (20% discount).
Aside from a few exceptions, we do NOT order labs using insurance. We have discounted rates with LabCorp and other specialty labs and pass these savings on to you. We pay for these labs and bill you for them as we are charged.

Initial Lab Work

COCFM’s Initial ReCODE Labs Panel contains more labs than are in the baseline Quest panel that you would get through Apollo Health.

There is certain Medical Equipment/Testing that we recommend for our clients. We can order these for you for the following cost:

In-Home Sleep Study (to rule out hidden sleep apnea) read by a physician certified in sleep medicine – $300
DexCom G6 Pro 10-day Continuous Glucose Monitor – Monitor with coaching visit for placement and coaching visit to review results – $200. DexCom G6 Pro Unit alone without coaching assistance – $100.

Personal MD - Concierge Communication

All COCFM clients have direct access to Dr. Dan LaPerriere through his private HIPAA compliant phone number. You can contact him via call or text with any health-related questions/concerns.

This means you have your doctor at your fingertips. You can request medication refills, ask questions about medications or supplements, get feedback on symptoms or concerns you may be having, etc…

In addition to his functional medicine knowledge, Dr. Dan has many years of experience in primary care, urgent care, and emergency medicine. He can handle a wide range of concerns that may present themselves.
