All of our ReCODE clients are also Concierge Wellness clients and take advantage of having Dr. Dan LaPerriere’s private number to reach out to him via call or text with any questions or concerns related to the ReCODE protocol or other health concerns. In addition to his functional medicine knowledge, Dr. Dan has many years of experience in primary care, urgent care, and in the emergency room.
Dr. Dan can be a resource for any health questions that may arise, he can take a look at a cut or rash (in the office or remotely), provide a second opinion when needed, act as a liaison between your various medical specialists, and help manage acute health concerns (as appropriate). If you are having a medical emergency you should go to the emergency room. COCFM is not your primary care office. We do not have acute care testing (ie strep tests, flu tests, COVID-19 tests, etc…), do not perform procedures such as biopsies or laceration repairs, and do not offer immunizations. If testing or hands-on intervention is required you may be advised/need to go to urgent care or the ER.
We provide world-class ReCODE and functional medicine care. We work closely with you to discover the underlying contributors to your cognitive issues and formulate/revise a plan to help stop and even reverse the progression of your dementia. You will be supported in learning to make the ReCODE lifestyle changes part of your daily life. We aim to provide you with as much assistance as you need for success. Different people will have different underlying issues; some may require deeper investigation and more visit time than others.
There is an Annual Membership Fee of $2,100
Clients must maintain an active ReCODE account with Apollo Health. This is NOT included as part of your COCFM membership.
Physician Rates
The time billed to you is all-inclusive. ReCODE work tends to involve a lot of extra time for Dr. Dan. Instead of billing for every minute spent providing care (preparing for your visit, reviewing and analyzing your labs, reviewing records, researching material for your case, the visit itself, developing your personalized plan, composing your visit summary, and answering follow up question), we bill a flat rate based on the face-to-face time spent together. Face-to-face time can be scheduled in person, via video visit, or via phone. Face-to-face time can be scheduled in person, via video visit, or via phone.
For each 30 minutes of face-to-face time, Dr. Dan typically spends an extra 30-60 minutes preparing for your visit, reviewing your labs, researching your case, developing and refining personalized recommendations, ordering follow-up studies, and composing your visit note.
Addition Family Members
You can add additional family members aged 16 or older.
The membership rate is $125 monthly for the 2nd individual and $100 monthly for each additional member beyond this.
* Pre-purchased physician and health coaching hours can be split among contracted family members
If a family member is doing PreCODE or ReCODE, they must maintain an active account with Apollo Health.
Health Coaching
Health coaching is extremely important if you wish to make lasting health changes and succeed in the ReCODE program.
We have a coach at COCFM that you can work with, but we are happy to support your work with any ReCODE 2.0 Certified health coach.
Optimally you would see your health coach at every other month during your first year in the program. However, as a COCFM ReCODE client, we expect you to meet with a ReCODE 2.0 Certified Health Coach at least every other month during your initial ReCODE year and at least 4 times a year in ongoing years.
Health Coach Rates
Prepay for 2 hours of face-to-face time ($115 per hour) = $230
Prepay for 4 hours of face-to-face time (100 per hour) = $300
Prepay for 6 hours of face-to-face time ($90 per hour) = $540
For your Initial Lab Work, you have the option of one of the following panels. Both of these panels contain more labs than are in the baseline Quest panel that you would get through Apollo Health.
