Doctor Dan LaPerriere is located in Louisville, in the NW Denver area metro, but is available for video conferences with clients in all the states in which he is licensed.
WHAT TO EXPECT:
This is an exploratory meeting where we will learn a bit about one another to see if we would be a good fit to work together. This time is not meant for lab or chart review and Dr. Dan will not be providing any specific medical advice.
This meeting can be in person. If you wish to meet in person, please call the office after you have scheduled your consultation to notify us.
There is a $100 non-refundable fee for booking this consultation. This will be applied to your membership cost if you sign a membership contract within 30 days of the consultation.
WHAT TO EXPECT:
You will spend your initial 30 minutes with Rachel (our RECODE 2.0 Certified Health Coach) where she will guide you through a MoCA test.
The second half of the consultation will be spent with Dr. Dan.
