All COCFM clients have direct access to Dr. Dan LaPerriere through his private HIPAA compliant phone number. You can contact him via call or text with any health-related questions/concerns.
This means you have your doctor at your fingertips. You can request medication refills, ask questions about medications or supplements, get feedback on symptoms or concerns you may be having, etc…
In addition to his functional medicine knowledge, Dr. Dan has many years of experience in primary care, urgent care, and emergency medicine. He can handle a wide range of concerns that may present themselves.
At COCFM you receive outstanding PreCODE and functional medicine care. We explore and correct underlying issues that may cause cognitive issues later in life and help give you the tools, knowledge, and support to help prevent cognitive decline and promote overall wellness. Different people will have different underlying issues and some may require deeper investigation and more visit time than others.
There is an Annual Membership Fee of $2,100
Physician Rates
Physician time is billed at an hourly rate . Billable/Deducted time includes all time spent preparing for your visit (review of records, review of questionnaires, review of labs), the visit time itself, time spent composing your visit summary/recommendations, and any follow up patient portal/email correspondence that requires more than 5 minutes of response time.
There is no additional charge for prescription refills or for brief correspondence (that takes less than 5 minutes of response time).
Time is billed in 10-minute increments.
Dr. Dan typically spends an extra 30-60 minutes time for each face-to face hour.
Addition Family Members
Additional family members 18 years of age and older can be added at the discounted membership rate of $50 per month for each person ($600 per year).
* Pre-purchased physician and health coaching hours can be split among contracted family members
If a family member is doing PreCODE or ReCODE, they must maintain an active account with Apollo Health.
Health Coaching
Health coaching is extremely important if you wish to make lasting health changes and succeed in the PreCODE program.
We have a coach at COCFM that you can work with, but we are happy to support your work with any ReCODE 2.0 Certified health coach.
Optimally you would see your health coach at least every other month during your first year in the program. However, as a COCFM PreCODE client, we expect you to meet with a ReCODE 2.0 Certified Health Coach at least 4 times during your initial year and at least twice a year in ongoing years.
Health Coach Rates
Prepay for 2 hours of face-to-face time ($115 per hour) = $230
Prepay for 4 hours of face-to-face time (100 per hour) = $300
Prepay for 6 hours of face-to-face time ($90 per hour) = $540
For your Initial Lab Work, you have the option of one of the following panels. Both of these panels contain more labs than are in the baseline PreCODE Quest panel that you would get through Apollo Health.
