Welcome to our Concierge Functional Medicine Program, where we aim to help you become the best version of yourself.

Our goal in working with you is to improve your health, wellness, energy, balance, and vitality and to prevent chronic illness.

Whether you are looking to address a n ongoing health issue, shed those stubborn 10 pounds, achieve a new personal best, eliminate brain fog, sharpen your mind, slow down the aging process, or prepare yourself for an amazing 90+ years of life, we are here for you every step of the way.

Personal MD - Concierge Communication

All COCFM clients have direct access to Dr. Dan LaPerriere through his private HIPAA compliant phone number.  You can contact him via call or text with any health-related questions/concerns. 

This means you have your doctor at your fingertips.  You can request medication refills, ask questions about medications or supplements, get feedback on symptoms or concerns you may be having, etc…

In addition to his functional medicine knowledge, Dr. Dan has many years of experience in primary care, urgent care, and emergency medicine. He can handle a wide range of concerns that may present themselves.

Individualized Concierge Functional Medicine Care

There is an annual membership fee of $2,100
($2,000 if paid upfront). Physician and Health coach time is an added cost. Prepaid hours can be purchased at a discounted rate.

There is an Annual Membership Fee of $2,100

  • This can be paid with monthly payments of $175, or you can pay $2,000 upfront and save $100.
  • This provides you with/covers:
    •  Concierge Wellness Care 
    • Portal access
    • Administrative costs
    • Quarterly computer-based cognitive assessments through Creyos
    • 20% off supplements purchased through COCFM
    • A significant discount on peptide/specialty medications purchased through COCFM
    • Discounted metabolic testing at COCFM

Physician Rates 

Physician time is billed at an hourly rate . Billable/Deducted time includes all time spent preparing for your visit (review of records, review of questionnaires, review of labs), the visit time itself, time spent composing your visit summary/recommendations, and any follow up patient portal/email correspondence that requires more than 5 minutes of response time. 

There is no additional charge for prescription refills or for brief correspondence (that takes less than 5 minutes of response time). 

Time is billed in 10-minute increments. 

Dr. Dan typically spends an extra 30-60 minutes time for each face-to face hour. 

  • Pay as you go = $450 per hour
  • Prepay for 4 hours of time ($425 per hour) = $1,700
  • Prepay for 6 hours of time ($400 per hour) = $2,400
  • Prepay for 10 hours of time ($375 per hour) = $3,750
  • Pre-purchased time does not expire. However, you need an active COCFM membership to use this time.

Health Coach Rates

  • Pay as you go = $125 per hour 

  • Prepay for 2 hours of time ($115 per hour) = $230

  • Prepay for 4 hours of time ($100 per hour) = $300

  • Prepay for 6 hours of time ($90 per hour) = $540

 
Other Information
 
  • Labs and Supplements are NOT included in your membership.
  • It is recommended that you purchase supplements through COCFM (20% discount) to ensure you are getting high-quality supplements.
  • Aside from a few exceptions, we do NOT order labs using insurance.  We have discounted rates with LabCorp and other specialty labs and pass these savings on to you.  We can tell you the approximate cost of labs before ordering them. We pay for these labs and bill you for them as we are charged.

Add additional 1st degree family members aged 16 or older for a discounted annual membership rate and share prepaid hours.

Rates for additional family members:

  • An initial person membership (above) is required
  • Add a second family member for $125 per month ( $1,500 for the year)
  • Add more family members (beyond the 2nd member) for the rate of $100 per month ($1,200 for the year) per person.
  • Pre-purchased physician and health coach time can be shared among family members time (see rates above).
  • Pre-purchased time does not expire. However, you need an active COCFM membership to use this time.
  • For new patients, a review of past records will be billed at half the normal rate ($250 per 30 minutes) in 15-minute increments, rounded to the nearest 15 minutes.

  • Labs and Supplements are NOT included in your membership.

Concierge Wellness Care

All COCFM members receive Concierge Wellness Care. Clients then chose from 3 programs: PreCODE, ReCODE, or Functional Medicine Care.
ReCODE + PreCODE Programs
Membership for ReCODE or PreCODE members. PreCODE is for preventive cognitive wellness while ReCODE is for treating cognitive decline.
